STEP 2 - Pass out the Enrollment Materials provided to you from Allegiance to all eligible employees -

STEP 3 - Return forms with elections made (note declinations can be housed in your employee files) to Allegiance. Allegiance offers many ways to send us your Enrollment forms:

STEP 4 - Once Allegiance receives and processes the forms a confirmation packet will sent to the Group contact. Depending on the options you elected your packet may or may not include the items listed below.
Employer Welcome Letter - Please read carefully for information regarding your plan.
Non Discrimination Testing - Please read instructions and complete and return as necessary.
Confidentiality Information - Please read instructions, distribute/fill out and return as necessary.
Invoice and List Bill - This is your first months list billing - please read information on funding and the list bill in your Employer Welcome Letter
Employee Confirmation Letters - Note this package will be mailed separately, please confirm and distribute to your employees.
