Open Enrollment - Mid Year Election Change, don't know what form to use? - To make any mid-year election changes or additions more convenient, we have developed a new Mid Year Election Change Form. To provide more efficient open enrollment, we have also built an on-line Enrollment Supply Request Form and improved the existing Enrollment Form.
